Payment and Billing

Outstanding balances may be sent to collections if not resolved within 90 days.
Patient Responsibilities
Patients are expected to provide accurate and complete medical history, follow treatment plans as prescribed, treat staff with respect, arrive on time for appointments, and notify us of any changes to contact or insurance information. Failure to comply with these responsibilities may affect the quality of care we can provide.
Limitation of Liability
While we strive to provide the highest quality healthcare services, we cannot guarantee specific outcomes. Our liability is limited to the extent permitted by law. We are not responsible for complications arising from failure to follow medical advice, pre-existing conditions, or factors beyond our control.
Dispute Resolution
Any disputes arising from these terms or our services will be resolved through binding arbitration in accordance with the rules of the American Arbitration Association. You agree to waive the right to participate in class action lawsuits. This arbitration agreement does not affect your rights under HIPAA or other patient protection laws.
